The Child and Adolescent Mental Health Service is based at Marlborough House, a purpose built unit located on site of the former Princess Margaret Hospital in Swindon. It provides a four tier multidisciplinary service to a catchment area with a population of approximately 300,000. Swindon is the main centre of population, a rapidly developing town with a good rate of employment. There are good road and rail links to Bristol (45 minutes), London (1 hour), Bath and Oxford. The DGH has moved to the new Great Western Hospital.
There are 1,500 referrals each year coming from a variety of Health, Education and other professionals. The outpatient service comprises a Generic Team and a Day Service for children up to the age of 12 years. There is a twelve-bedded Adolescent Inpatient Unit which serves a catchment area of most of Wiltshire, Swindon and Gloucestershire. Peripheral clinic sessions are held in Tetbury, Marlborough, and Ludgershall. Community based parenting groups are co-worked with Health Visitors and a training programme is offered for primary care workers in running parenting groups for parents of pre-school children. There are strong links with Paediatrics, Education and Social Services with the opportunity for liaison and consultation work.
All four Consultants are actively involved in management and planning, both within the Trust and between the local agencies, and Specialist Registrars would have the opportunity to participate in these activities. Within the service there are weekly management meetings through a monthly cycle, each with different functions around budget setting, audit, service management and linking between the different parts of the service. On Wednesday mornings there is an in-service training seminar along with a team co-supervision slot.
Each of the separate parts of the service has their own individual team meetings in order to allocate and prioritize clinical work. Within the Specialist Services other disciplines making up the team include Clinical Psychology, Family Therapy, Child Psychotherapy, Art Psychotherapy, Occupational Psychotherapy and Behavioural Group Therapy. Specialist teaching staff from Swindon Education Authority work permanently within the service.
Optional additional experience in Learning Disability
An opportunity of attachment to the Learning Disabilities Service, a multidisciplinary team providing services for children and adults is available. The Supervising Consultant is Dr Chris Speller.
Special interests
The Specialist Registrar will be expected to develop special interests in clinical practice. Given the comprehensive nature of the local service, it is anticipated that most special interests could be catered for.
Clinical training and meetings
There are opportunities for clinical training on a regular basis. The Child and Adolescent Mental Health Service meets weekly for case discussions and seminars. Less frequently there are meetings of Child Psychiatrists with an academic component.
Supervision
A Child Psychotherapist will provide supervision of individual psychotherapy work. A Family Therapist will provide supervision of family therapy. A personal tutor will be available to the trainee, with weekly supervision.
Library facilities
A small but comprehensive library and key journals are available at Marlborough House. Improved library facilities and a new education centre are available at the Great Western Hospital, Swindon, and the University library in Bristol.
Audit
The service is committed to Clinical Audit and the Specialist Registrar is expected to be actively involved in this work.
Research
Research will be encouraged and time set aside in the programme for this purpose. Advice and supervision will be made available from an experienced Consultant in the field.
Teaching
The Specialist Registrar will be expected to be actively involved in the teaching of medical students, junior trainees in Psychiatry, trainees in Paediatrics and other disciplines.
Management and Planning
The service has a management group of which the Specialist Registrar may be an observing member. It is responsible for its own planning and budget, and accountable to the Clinical Director, Dr Janet King (Consultant Paediatrician) in the Directorate of Child Health.
