The post is based at Marlborough House Adolescent Inpatient Unit and the postholder will work with Dr Richard Eyre and Dr Don Batton. Marlborough House offers a Tier 4 Service for Adolescents with severe psychiatric disorder from Swindon, Wiltshire and Gloucestershire. There are 12 in-patient beds and 6 day attender places. Referrals come from Tier 3 CAMHS across the catchment areas. The range of clinical presentations includes psychosis, major affective disorders, eating disorders, severe and disabling anxiety disorders (including OCD, agoraphobia and school refusal), Neurodevelopmental disorders, PTSD and sever attachment and relationship difficulties. The postholder will be able to participate in the weekly Multi-disciplinary Assessment Clinic. New referrals are seen with their families by the Team during a full morning, with the aid of one-way screen facilities. Whole family and individual interviews are followed by multi-disciplinary discussion, before a final feedback to the young person and family. The postholder will have role in the admission, examination, investigation and treatment monitoring and planning of young people admitted to the Unit. The postholder will participate in Clinical Meetings (equivalent to “Ward Rounds”) for each patient, and collate initial Psychiatric Summaries for patients 2 – 3 weeks post admission. The postholder may participate in Reviews – the 6 weekly progress meeting with patient, family, referrer and multi-disciplinary team which summarised progress and refines the Careplan – where appropriate leading to discharge planning and CPA handover to the referring local team. Clinical treatments are varied and complementary, including pharmacotherapy, cognitive behavioural therapy, family therapy, key-nursing individual work, occupational therapy, group work – along with the therapeutic role of the milieu. The postholder will have the opportunity to experience and understand these therapeutic approaches, and where appropriate receive specific supervision for a cognitive behavioural or behavioural therapy and join the Unit Family Therapy Clinic to follow a case(s) through a treatment course.
