This post is primarily based on Acer Unit, which is a dedicated 6-bed substance misuse inpatient unit at Callington Road Hospital and the postholder will work with Dr Fergus Law. The main work of the unit is the stabilization or detoxification of patients dependent on either opiates or alcohol. Patients are usually admitted on Mondays for a fixed 2-week period. The postholder will be responsible for pre-assessment of patients coming to the unit (usually the week before) and then for providing their ongoing medical care during the admission. There are also many opportunities to extend the experience, through, for example, the outpatient substance misuse work at Blackberry Hill Hospital and Stokes Croft, the outpatient alcohol service at the Robert Smith Unit and through an ongoing and active clinical research program.

This post is based at 44 London Road, Gloucester and the postholder will work with Dr Karen Williams and the West Gloucestershire Substance Misuse Team. The post occupied by the doctor for the third year of specialist training will provide an introduction to Substance Misuse Psychiatry.  The post will enable the doctor to gain core skills in the assessment, management and treatment of patients with drug and alcohol problems in both outpatient and inpatient settings. In addition the doctor will have the opportunity to gain experience in psychosocial treatment of Substance Misuse Disorders in group and individual settings.   Outpatient clinics will be supported by an experienced clinician. The post holder will also work with the specialist inpatient service in Cheltenham to provide Medical and Psychiatric assessment and treatment of people on detoxification or stabilisation programmes. Clinical supervision will be provided by the consultant and other senior clinicians. The doctor should have the competencies required to manage a small case load by the end of their training and should also have met the educational needs required for the MRCPsych examination section on Substance Misuse Psychiatry. The special interest session in Forensic Psychiatry will provide opportunities for the assessment and management of patients referred for low secure care and mentally disordered offenders in contact with the criminal justice system. After a short induction period the training in Substance Misuse Psychiatry will require the doctor to work alongside professionals in the multi-disciplinary team to deliver outpatient assessment and management of patients with drug and alcohol problems. Skills required to assess people with Substance Misuse disorders will be gained by supervision with their educational supervisor and joint work with other professionals in the service. As their competency increases they will take on a small case load and follow these patients through the course of their placement in Substance Misuse Psychiatry.

This post is based at 47 Boulevard, Weston-super-Mare and the postholder will work with Dr Sam Forshall. There are 3 components to the post: to provide inpatient care to Dr Forshall’s patients on Juniper Ward, Long Fox Unit, including admission, care while on the ward and discharge arrangements; to provide medical input to the Community Drug & Alcohol Team, including keyworking and prescribing and mental health assessments; to provide on call cover to the Long Fox Unit as a whole on a rota basis. The ST will be expected to clerk all new admissions of Dr Forshall’s patients to Juniper Ward, to order appropriate investigations, and to be responsible for their day-to-day physical care, in association with the other Doctors on the unit. The ST will be expected to keep good medical records, document Core Assessments, to update treatment plans, and to be involved in Care Planning meetings and discharge arrangements. The ST is responsible for discharge summaries and appropriate letters. After an initial induction, the ST will be the main prescribing Doctor for the Rapid Access Clinic. The ST will hold a case load up to 10 patients and will hold keyworking responsibility for those patients. In addition the ST will provide prescribing assessments and mental health reviews for patients held by the team. Supervision will be provided by Dr Forshall. The ST will be part of the day time on call rota covering the Long Fox Unit and its inpatient beds. In addition the ST when on call will provide medical support to both the Crisis and Home Treatment Team as well as the Liaison Team covering Weston General Hospital.

This post is based at Green Lane Hospital, Devizes and the postholder will work with Dr Julie Hankin. The ST doctor would work alongside the consultant and team in providing community and in-patient care to these patients. The team has use of up to 3 in-patient beds on Imber Ward, Green Lane Hospital. The team aims to take a bio-psychosocial approach to treatment and works primarily with patients who have a severe and persistent mental disorder associated with a high level of disability, complex problems and a history of poor engagement with services and high use of in-patient or home-based care. The ST doctor will be expected to see all new admissions and complete a thorough history and examination, including physical examination. He/she will be expected to dictate an initial history, examination, formulation and plan of treatment within one week of the patient's admission. He/she will attend the consultant ward conferences. The ST doctor will be responsible for regularly updating the patient's treatment plan during their stay. They will ensure that discharge and after-care plans are clearly detailed prior to a patient's discharge and that a discharge notification form for the GP is completed. A discharge summary should be dispatched to the GP within ten working days of the patient leaving hospital. The ST doctor will attend a once weekly out-patient clinic in Calne, seeing a new case in consultation with the consultant, and following up other patients. Liaison with, and advice to, members of the community team. Participation in a weekly clinical liaison meeting and the multi-disciplinary discussion of the management of cases. Attendance at Day Hospital meetings and liaising with day hospital staff in Chippenham. Occasional assessments of urgent referrals from the catchment area.

The post is based at Marlborough House Adolescent Inpatient Unit and the postholder will work with Dr Richard Eyre and Dr Don Batton. Marlborough House offers a Tier 4 Service for Adolescents with severe psychiatric disorder from Swindon, Wiltshire and Gloucestershire. There are 12 in-patient beds and 6 day attender places. Referrals come from Tier 3 CAMHS across the catchment areas. The range of clinical presentations includes psychosis, major affective disorders, eating disorders, severe and disabling anxiety disorders (including OCD, agoraphobia and school refusal), Neurodevelopmental disorders, PTSD and sever attachment and relationship difficulties. The postholder will be able to participate in the weekly Multi-disciplinary Assessment Clinic. New referrals are seen with their families by the Team during a full morning, with the aid of one-way screen facilities. Whole family and individual interviews are followed by multi-disciplinary discussion, before a final feedback to the young person and family. The postholder will have role in the admission, examination, investigation and treatment monitoring and planning of young people admitted to the Unit. The postholder will participate in Clinical Meetings (equivalent to “Ward Rounds”) for each patient, and collate initial Psychiatric Summaries for patients 2 – 3 weeks post admission. The postholder may participate in Reviews – the 6 weekly progress meeting with patient, family, referrer and multi-disciplinary team which summarised progress and refines the Careplan – where appropriate leading to discharge planning and CPA handover to the referring local team. Clinical treatments are varied and complementary, including pharmacotherapy, cognitive behavioural therapy, family therapy, key-nursing individual work, occupational therapy, group work – along with the therapeutic role of the milieu. The postholder will have the opportunity to experience and understand these therapeutic approaches, and where appropriate receive specific supervision for a cognitive behavioural or behavioural therapy and join the Unit Family Therapy Clinic to follow a case(s) through a treatment course.

This post is based at Delancey Hospital, Cheltenham and the postholder will work with Dr Rosemary Richards. The post occupied by the doctor for the third year of specialist training will provide an introduction to Child and Adolescent Psychiatry. Specialist Child and Adolescent Mental Health Services (CAMHS) work is in out-patient clinics with great opportunities for multi-disciplinary experience. The post is in Cheltenham with responsibility for a locality extending to Fairford, Moreton-in-Marsh, Cirencester and Lechlade. We have a very varied range of work from pre-school to 18 years with all types of pathology. We always have a few in-patients (out of county) so that tier 4 provision may be seen. Experience in medication use can be gained with ease and is well supported. The ST doctor has a caseload tailored to individual learning needs. Clinical supervision will be provided by the consultant and other senior clinicians. The doctor should have the competencies required to manage a small case load by the end of their training and should also have met the educational needs required for the MRCPsych examination section on Child and Adolescent Psychiatry. A special interest session in Learning Disability affords an opportunity to gain experience in assessment and management together with an understanding of general provision. The training in Child and Adolescent Psychiatry will require the doctor to shadow various professionals in the multi-disciplinary team and to learn the skills required to assess children and families with a broad range of presentations from a developmental perspective with their educational supervisor.   As their competency increases they will take on a small case load and follow these patients through the course of their placement in Child and Adolescent Psychiatry.

This post is based at Montpelier Low Secure Recovery Unit, Wotton Lawn Hospital, Gloucester and the postholder will work with Dr James Laidlaw. The post occupied by the doctor for the third year of specialist training will provide an introduction to forensic psychiatry.  The post will enable the doctor to gain core skills in the assessment and treatment of patients referred for low secure care and mentally disordered offenders in contact with the criminal justice system. In addition the doctor will have the opportunity to learn the skills required in the assessment of risk of violence and an understanding of the interface between forensic and general adult psychiatry. The doctor will also work closely with the MU multi-disciplinary team, the HMP Gloucester Psychiatric Inreach Team and the Gloucestershire Criminal Justice Liaison Service. Clinical supervision will be provided by the consultant and other senior clinicians. The doctor should have the competencies required to manage a small case load by the end of their training and should also have met the educational needs required for the MRCPsych examination section on Forensic Psychiatry. The training in forensic psychiatry will require the doctor to shadow various professionals in the multi-disciplinary team and to learn the skills required to assess people with mental disorder and offending behaviour with their educational supervisor. As their competency increases they will take on a small case load and follow these patients through the course of their placement in Forensic Psychiatry. The special interest session in Substance Misuse Psychiatry will involve a clinic run jointly with an experienced member of the West Gloucestershire Substance Misuse Team and a session on the specialist Addiction ward where the post-holder would be expected to clerk in planned admissions to the ward for drug and alcohol detoxification and drug stabilisation.

This post is based at Sandalwood Court, Highworth Road, Swindon and the postholder will work with Dr Malti Sarin and the Crisis and Home Treatment Teams. Provision of comprehensive psychiatric care through evaluation, diagnosis and use of appropriate treatment plans/strategies. Providing support and clinical assistance to the consultant psychiatrist with regard to the care of patients. This will include management of patients through home visits, CPA and outpatient clinics. Assessing patients in the community, Great Western Hospital and the Police Station. Ensuring the involvement of families and carers, wherever possible, with the patients consent in treatment planning. Providing clinical advice, along with other senior medical staff, to the Swindon Crisis Team. This includes clinical case discussions, treatment planning to meet the patients needs and regular reviews of such plans. Liaising with other doctors, wards, and outside agencies, as may be appropriate, especially with regard to continuity of patient care. Participating in research, clinical governance, clinical audit, medical advisory committee and other committees as may be necessary. The ST will be expected to see all new admissions and complete a thorough history and mental state examination plus physical examination. He/she will be expected to dictate an initial history, examination, formulation and plan of treatment within one week of the patient's admission. He/she will attend the consultant ward conferences. The ST will be responsible for regularly updating the patient's treatment plan during their stay. They will ensure that discharge and after-care plans are clearly detailed prior to a patient's discharge and that a discharge notification form for the GP is completed. A discharge summary should be dispatched to the GP within ten working days of the patient leaving hospital. This post is based at Green Lane Hospital, Devizes and the postholder will work with Dr Peter Crofts. There is a Community Mental Health team base at Red Gables, Trowbridge. Dr Peter Crofts is Consultant providing General Adult Psychiatric services to the town of Melksham together with the surrounding villages. There are supporting day hospitals at Red Gables, Trowbridge. The main responsibility of the post holder will be the in-patients on Imber ward, Green Lane Hospital, but there will be an expectation and encouragement for the post holder to become involved with other disciplines within the community team. The ST3 will be expected to see all new admissions and complete a thorough history and examination, including physical examination. This will be documented in the notes and on attendance at the Consultant ward round. The ST3 will be responsible for regular records in patient's notes. The ST3 will be expected to visit the acute wards daily and see all acute in-patients at least once a week in addition to the Consultant ward conferences. The ST3 will be required to know what is happening on the ward and the contributions made by all members of the multi-disciplinary team. The ST3 will be responsible for informing the patient's GP immediately on discharge and dictating a discharge summary within two weeks of the patient leaving hospital. The ST3 will be involved with two weekly out-patient clinics at Red Gables, Trowbridge. They will be expected to see one new referral per clinic and to present this in a standard format to the Consultant followed by discussion and will also follow up a cohort of their own patients. The opportunity to accompany the Consultants on domiciliary visits will be encouraged. The opportunity to attend care programme approach meetings and other multi-disciplinary meetings will be encouraged. The opportunity to visit other (non-statutory) agencies involved in mental health care provision will be encouraged.

This post is based in Stapleton, Bristol and the postholder will work with Dr Leila Cooke. Core clinical work: Mainly out-patient work in clinics (max two per week) and in residential homes, sometimes in day centres and schools; joint assessments with members of the team; liaison with carers and general practitioners with advice on management; In-patient work in 12 bedded Assessment and Treatment Unit in Warmley (approx 1-2 patients at a time); Fortnightly CLDT Meeting. Learning Objectives: Assessment and treatment of psychiatric disorders and epilepsy in learning disability; To gain knowledge of developmental disorders especially autism; To gain knowledge about the learning disability service and work closely with the community team; To improve communication skills. Academic activities: Fortnightly Peer Support Group facilitated by ST4-6 trainees; Monthly journal club, case conference, topic review and audit for medical staff; Monthly multidisciplinary academic conference-topics based on curriculum for psychiatric trainees; Attendance at weekly MRCPsych course; Research/Audit/Special interest session if part of educational plan.

This post is based at Heathfield and the postholder will work with Dr Heena Hargoven/Dr Chikkanna Manju/Dr Mark Scheepers/Dr Paul Winterbottom and Dr Karen Poon. The post occupied by the doctor for the third year of specialist training will provide an introduction to the Psychiatry of Learning Disability. The post will enable the doctor to gain core skills in the assessment and treatment of patients suffering from mental illness, challenging behaviour and epilepsy. In addition the doctor will have the opportunity to learn the skills required in the diagnosis of pervasive developmental disorders and an understanding of the impact having a learning disability has on people’s everyday function. The doctor will be mainly community based and work closely with the multi-disciplinary team. Clinical supervision will be provided by the consultant and other senior clinicians. The doctor will have a choice to work with the different trainers and will be given the opportunity to set up their educational (competency) needs with the trainer best able to achieve these. The doctor should have the competencies required to manage a small case load by the end of their training and should also have met the educational needs required for the MRCPsych examination section on Learning Disability. Arrangements locally have been negotiated with the CAMHS consultant allocated the CAMHS trainee to have the ST3 trainee attend a special interest session for the duration of their placement in LD. This would provisionally be one session per week, but dependent on the trainee’s needs and personal development aims, this could be increased to two sessions. The training in the Psychiatry of Learning Disability will require the doctor to shadow various professionals in the multi-disciplinary team and to learn the skills required to assess people with a learning disability with their educational supervisor. As their competency increases they will take on a small case load and follow these patients through the course of their placement in the Psychiatry of Learning Disability. The special interest session in Child and Adolescent Psychiatry will need to be the trainee’s responsibility to set up once they have arrived into post and depending on their educational commitments etc, will be released from LD duties one or two sessions per week following discussion with their consultant supervisor.

Learning Disability Psychiatry

Dr Sheila Frazer

This post is based at Burton Place, Taunton and the postholder will work with Dr Sheila Frazer. This is a community based post which gives experience of assessment and treatment of adults with learning disabilities. In addition to mental health issues, referrals are also accepted for challenging behaviour and epilepsy. The post is based across two Community Adult Learning Disability Teams, Taunton Deane and Somerset Coast. Team members include psychologists, community nurses, speech and language therapists, physiotherapists, occupational therapists, social workers and psychiatrists. Admissions to acute mental health units are infrequent but some inpatient work is offered at these times. The postholder will manage their own diary, planning clinics independently.
